- Connected Health Resources supports patients and families in health care transition, medication management, treatment and recovery.
- The Patient and Caregiver Gateway (the primary service) delivers plain-language and multilingual tools and resources to patients and family caregivers that improve engagement after acute-care facility discharge and promote integration of healthcare, social services, and community supports -- improving patient outcomes.
- CHR Pilot Project. Our first pilot project for Connected Health Resources has begun in Montgomery County, Maryland. The Community Health Gateway helps patients and their families manage their health and services needs by providing information about community resources, maintaining medication information and supporting the Medication Therapy Management program provided by Alfa Specialty Pharmacy.
